Independent Laboratory HCFA Claim Filing Requirements  
As previously communicated in the June and October 2012 issues of Network Update, Empire BlueCross BlueShield has fully implemented the Blue Cross and Blue Shield Association (BCBSA) claim filing requirements for ancillary provider types.  
Specific to independent laboratory providers billing on a HCFA 1500, the claim must show the referring physician’s information, including name and NPI in field 17B. If the referring information is invalid or blank, the claim will be rejected.
