Having an accurate health record can be critical to ensuring you get safe, effective treatment. By using our unique online tool, MyHealth Record, you can easily maintain and track your personal health information along with your medical claims and have access to it 24 hours a day, whenever it’s convenient for you.  
Now’s the perfect time to organize your health record because we’ve added new features that can give your doctors a comprehensive health history to help with the planning of your care, avoid the potential for adverse reactions and eliminate duplication of services.

Here’s what’s new:

You can communicate with your doctors via MyHealth Record’s Secure Message feature. Ask your doctors a question or seek advice related to a condition, surgery, test, medication, or more. Your doctor can respond back to you personally through the secure message center.

Your doctor can add his or her own notes. Just grant your doctor permission to access your record, and then he or she can add personal notations to specific entries. You’ll be able to view the notes through the summary/review section and print it out so you can discuss the notes at your next office visit.

 
And here’s a sample of what you can do with MyHealth Record:

Add information to create a complete medical history by including dates of tests and procedures, immunizations, a list of any allergies and any prescription and over-the-counter drugs you are taking.

Track doctor visits and other well-care services.

Print and share your summarized medical history with your doctors.

By granting Empire authorization we can do some of the work for you. We can populate your record using data currently on file and update your record daily with information based on doctor visits, filled prescriptions, lab results and other claim information.*

 
How to access MyHealth Record: Just log in to Member Online Services. It’s easily accessible through your private, secure home page at MyHealth, powered by WebMD.  
 
Register for MyHealth Record and grant your doctors access to it today.
Remember, the more information your doctor has at his or her disposal, the better informed you’ll both be when making health care decisions.

To access MyHealth Record: 

1. Log in to Member Online Services and click on MyHealth.

2. Click on MyHealth Record.

MyHealth Record is available as part of 360° Health®, Empire's health services program.  

* The types of data supplied by Empire will vary depending on your health plan and may not be complete (for instance, claims that may not have been received or posted by Empire).
