Qualified children under the age of 19 can have their very own health insurance plan, New York State's Child Health Plus.
Eligibility
New York State residents residing within our 28-county service area
Under 19 years of age
Not eligible for Medicaid
Not covered by a comparable healthcare plan
Children are not eligible to enroll in Child Health Plus if a parent or family member is a public employee with access to family coverage through a state health plan that is paid for by the agency in whole or in part.
Benefits
Benefits are available for in-network services only with the exception of emergency room services. Covered services include:
Inpatient hospital services consisting of daily room and board, general nursing care, special diets, miscellaneous hospital services and supplies
Inpatient mental health, alcohol and substance abuse services
Emergency care provided in an emergency room of a participating or non-participating hospital
Pre-admission testing
Physician services provided by your child's primary care physician (PCP) or a network referral specialist with a valid referral from your child's PCP
Preventive healthcare and immunizations in accordance with the schedule established by the American Academy of Pediatrics
X-ray and laboratory services
Rehabilitative therapy
Radiation therapy and chemotherapy
Obstetrical and gynecological services
Outpatient visits for mental health and the treatment of alcoholism and substance abuse
Second surgical opinions
Speech and hearing services
Eye examinations and prescription corrective lenses
Then submit the materials along with proof of income, proof of residence and age of your children and proof of immigration (when applicable)

A completed application received by the 20th of the month will be effective the first of the following month.
Your Cost
Your cost for the Child Health Plus program will be determined by your household's total gross (pretax) monthly income.
If you qualify for subsidized coverage, your coverage may be free or your premium may be reduced. You can estimate your premium using the information in the Child Health Plus Premium Chart
If your income exceeds the subsidized limit, you may purchase CHP at the current monthly premium rate.
Final premium determination will be made based on the information supplied with your application and documentation.
Your first month's premium contribution must be included with your application.
