Direct Pay HMO/POS
Empire's Direct Pay HMO/POS plan offers the health plan benefits of both a traditional HMO (Health Maintenance Organization) for in-network services and the additional freedom to obtain out-of-network POS (Point of Service) medical services.   

In-patient and out-patient coverage

Primary Care Physician (PCP) provides or coordinates most care

Emergency care is always available at the nearest doctor or hospital

 
Direct Pay HMO
Empire's Direct Pay Health Maintenance Organization (HMO) allows you to select a Primary Care Physician (PCP) to provide basic and preventive care, give you referrals to specialized care, maintain your medical history, arrange admissions to hospitals and pre-certify special services—all within Empire's HMO network.   

Pharmacy benefits are included in this individual health plan

TraditionPLUS Hospital Program 
Covers your hospital needs-and the hospital needs of your covered dependents-even when you are far away from home.  
 
Healthy New York  
Provides affordable health coverage to eligible individuals and sole proprietors. 
 
Child Health Plus plan  
Qualified children under the age of 19 can have their very own health insurance plan, New York State's Child Health Plus.
