Covers your hospital needs-and the hospital needs of your covered dependents-even when you are far away from home.  
Eligibility 
A New York State resident residing within our 28 county service area.
Not eligible for group health insurance coverage.
Between the ages of 18 and 65 and not eligible for Medicare. If you are under age 19, you may be eligible to enroll in our Child Health Plus plan.
 
Benefits 
365 days of paid-in-full inpatient hospital benefits for covered services at a participating hospital.
100% coverage for in network preventive care services
Inpatient hospital semiprivate rooms and board (paid in full in participation hospitals).
Full range of hospital services, facilities, equipment and supplies.
Maternity care in birthing centers or hospitals.
Hospital care for newborns, up to 30 days.
Inpatient physical therapy and rehabilitation, up to 30 days.
Inpatient behavioral healthcare, up to 30 days.
Emergency care for sudden, serious illness or accidental injury.
Outpatient surgical facility charges.
Pre-admission testing.
