What Happened? What We Are Doing.

We learned on September 1, 2016 that an individual in the Medicare sales department emailed company information to his personal email address between February and September of this year. His actions violated our policies on the accessing and handling of protected health information and personally identifiable information. The information emailed included certain elements of personal information, including names, dates of birth, addresses, health plan information and, in some cases, Medicare ID numbers. We directed the individual to securely destroy the information that was not properly in his possession, and steps were also taken to confirm the destruction of the information. The individual is no longer employed with our company. When questioned, the individual advised that he was using the data to validate his commission payments. The information obtained by the individual is the property of the company, and he, like all employees, was prohibited from sending such information outside of the company. At this time, we have no reason to believe that the information is at risk for financial misuse or identity theft.

We are committed to protecting the privacy and security of individuals’ protected health information and personally identifiable information.

Who is Being Notified? How we are Working to Protect You.

We have worked diligently since discovery of this matter to identify all individuals whose information may have been impacted by the actions of the former sales employee. We have identified approximately 3,500 Medicare-eligible individuals impacted and we are in the process of contacting these individuals for whom we have valid addresses by U.S. Postal Service addresses. For those whose Medicare ID numbers (which may include a Social Security number) may have been included, we will offer free identity theft protection and credit repair/monitoring services through AllClear Credit and Identity Theft Monitoring and AllClear Identity Repair. Those with questions can contact AllClear ID at 1-855-260-2766 Monday - Saturday, 9 a.m. - 9 p.m. Eastern Time.
