Empire Dental Plans


Empire Dental Pediatric Plan


The Empire Dental Pediatric PPO plan provides pediatric dental Essential Health Benefits for children (age 18 and younger) as required by the Affordable Care Act. This coverage is already included in many health plans. But if yours doesn’t, you can get it right here. This plan covers:

  • Cleanings, exams, X-rays: You pay 25% in-network / 25% out-of-network
  • Fillings: You pay 25% in-network / 25% out-of-network
  • Crowns, root canals, oral surgery: You pay 50% in-network / 50% out-of-network
  • Orthodontia: You pay 50% in-network / 50% out-of-network when medically necessary
  • Out-of-pocket limit: You pay no more than $350 per child, $700 for two or more children
  • Access to any dentist: You save money by visiting a dentist in our network.

As soon as your coverage starts, you can get your kids the dental care they need — no waiting periods! And there’s no limit per year on what this plan pays for covered dental care for your kids.

Empire Dental Family Plans


Our Empire Family PPO plans provide dental coverage for the whole family – kids and adults. For kids (age 18 and younger) this plan provides pediatric dental Essential Health Benefits as required by the Affordable Care Act. See “Empire Pediatric Dental Plan” above for details. For adults (age 20 and older), the Empire Dental Family plan covers:

  • Cleanings, exams, X-rays: You pay 0% in-network / 50% out-of-network
  • Fillings: You pay 50% in-network / 50% out-of-network, after a 6 month waiting period
  • Crowns, root canals, oral surgery: You pay 50% in-network / 50% out-of-network, after a 12 month waiting period
  • Access to any dentist: You save money by visiting a dentist in our network.

Empire Dental Family Enhanced Plans


Our Family Enhanced PPO plans provide dental coverage for the whole family – kids and adults. For kids (age 18 and younger) this plan provides pediatric dental Essential Health Benefits as required by the Affordable Care Act. See “Empire Pediatric Dental Plan” above for details. For adults (age 20 and older), the Empire Dental Family Enhanced plan covers:

  • Cleanings, exams, X-rays: You pay 0% in-network / 50% out-of-network
  • Fillings: You pay 20% in-network / 50% out-of-network, after a 6 month waiting period
  • Crowns, root canals, oral surgery: You pay 50% in-network / 50% out-of-network, after a 12 month waiting period
  • Access to any dentist: You save money by visiting a dentist in our network.
  • Cosmetic Orthodontia for kids: You pay 50% in-network / 50% out-of-network, after a 12 month waiting period with a lifetime maximum of $1,000

Dental Prime Plans for Individuals and Families


Our Dental Prime PPO plans provide dental coverage for the whole family – kids and adults. There are three plan options (plans A, B, and C) which provide coverage ranging from just cleanings, exams and X-rays to more comprehensive coverage including fillings plus crowns, root canals, and oral surgery. Dental Prime plans cover:

  • Cleanings, exams, X-rays: You pay 0% in- and out-of-network (Plans A, B and C)
  • Fillings: You pay 20% in- and out-of-network, after a 6 month waiting period (Plans B and C)
  • Root canals and oral surgery: You pay 50% in- and out-of-network, after a 12 month waiting period (Plans B and C)
  • Crowns, dentures and bridges: You pay 50% in- and out-of-network, after a 12 month waiting period (Plan C)
  • Access to any dentist: You save money by visiting a dentist in our network.

The Dental Prime plan also offers:

  • Extra cleaning or periodontal service if you’re pregnant or diabetic (Plans A, B and C)
  • Brush biopsy to help detect any early signs of oral cancer (Plans B and C)

Dental Prime does not provide the pediatric dental Essential Health Benefits as required by the Affordable Care Act.

Empire Vision Plan


Blue View Vision is easily added to any Empire health and/or dental plans. It covers:
  • Eye exams every 12 months ($20 copay)
  • New frames every 24 months ($130 allowance)
  • Standard lenses every 24 months — single, bifocal and trifocal ($20 copay)
  • Contact lenses every 24 months — conventional and disposable ($80 allowance)
