A care plan is a detailed approach to patient care that is customized to a patient's needs, goals, and preferences. In order for care plans to be successful, the patient must be an engaged partner in the care planning process.
In order to have an effective population-based care delivery system, practices will need to access data and analyze their patient population to identify care opportunities and target audiences who may be at higher risk or who need additional support.
