Employer Responsibilities

An employer’s general responsibilities with respect to Anthem Life & Disability Insurance Company’s (Anthem Life) group plan include:   

giving notice of eligibility to each employee who is or will become eligible for enrollment;

obtaining applications for eligible employees wishing to enroll for coverage;

submitting all applications, notices, or other information or inquiries received from eligible employees or, if billing is ”self-administered,” maintaining all applications, notices, or other information or inquiries received from eligible employees;

distributing certificates and other information to covered employees;

paying premiums on or before the premium due date, even if covered employees are required to contribute toward the premium;

maintaining an insurance records file for each employee, along with any changes to classification, benefit amounts, beneficiary and other relevant details (we may periodically request information that would be contained in this insurance file);

reporting or tracking employee changes and their effective dates (changes include name, class, employment status, dependent status, and earnings if benefit amounts are salary-based);

helping covered employees file claims;

notifying employees of their conversion rights and any portability privileges upon termination of employment or coverage;

reporting taxes withheld from disability benefits; and

producing W-2’s for any disability benefits received by employees under short term disability or long term disability insurance.
